MEMBERSHIP DUES AND FORMS

For RETURNING Members, in addition to making your dues payment, if there has been ANY CHANGE in contact information or medical emergency information, we require updated forms be resubmitted. We also require updated Babysitter Designation forms be submitted. If all information remains the same as last season and you do not plan on using a Babysitter or Nanny, simply complete your payment and you are set!


If you are a first year Member, you will need to complete and mail in all forms below and make dues payment under NEW MEMBER section in the cart.
